Membership benefits depend on the plan selected and may include routine dental evaluations, clinically necessary routine dental X-rays, preventive hygiene care, fluoride or nano-hydroxyapatite varnish, and other specifically listed benefits.

Membership also provides 20% off eligible dental procedures performed and billed by 208 Dental, unless a service is specifically included or excluded under the plan.

Included benefits are subject to the quantities and limitations stated for the selected membership plan.

Unless specifically included or excluded, eligible dental procedures performed and billed by 208 Dental receive 20% off the current regular office fee.

Eligible procedures may include:

• Fillings and buildups
• Crowns and bridges
• Root canals
• Extractions and oral surgery
• Implants
• Dentures and partials
• Sealants
• CBCT/3D imaging when charged
• Nitrous oxide
• In-office professional whitening
• Eligible cosmetic procedures
• Orthodontic treatment performed by 208 Dental
• Other eligible restorative, surgical, and in-office treatment

There is no annual dollar maximum on eligible treatment receiving the membership discount.

The Membership Savings Plan does not impose insurance-company frequency rules or waiting periods on clinically appropriate eligible treatment simply because you recently had a similar procedure.

Included preventive and diagnostic benefits are subject to the quantities stated in the selected plan.

The Periodontal Care Plan is $429 per year and is designed for patients who need more frequent professional hygiene care. The plan includes 3 included hygiene visits per membership year.

Depending on the patient's diagnosis and clinical needs, D1110, D4910, D4346, or D4355 may each use one of those three included hygiene visits.

If D4346 (scaling in the presence of generalized moderate or severe gingival inflammation) or D4355 (full-mouth debridement) is clinically appropriate, the procedure uses 1 of the 3 included hygiene visits. These procedures are not automatically billed separately at the 20% membership discount.

Scaling and root planing (D4341/D4342) is not included as one of the three hygiene visits. SRP receives the 20% membership discount when performed and billed by 208 Dental.

Following scaling and root planing, periodontal maintenance visits (D4910) may use the patient's included hygiene visits according to the recommended maintenance schedule.

The dental team will recommend the appropriate treatment and hygiene schedule based on the patient's individual clinical needs.

A qualifying patient who completes a New Patient Special may enroll in the Membership Savings Plan within 30 days of that visit and apply the amount actually paid for the special toward the annual membership fee.

• The New Patient Special visit date becomes the membership effective and anniversary date.
• Services already provided through the special count toward the first membership year's included benefits.
• After 30 days, the patient may still enroll but must pay the full current membership fee.
• The New Patient Special and Membership Plan are not stacked as two separate discounts.

For the Adult New Patient Special, the cleaning means a routine adult prophylaxis (D1110), when clinically appropriate. It does not include periodontal treatment such as D4346, D4355, D4341, D4342, or D4910.

Membership fees are non-refundable and non-transferable once paid, except where otherwise required by applicable law.

This remains true if the patient:

• Obtains dental insurance during the membership period
• Chooses to receive care at another dental office
• Relocates
• Chooses not to use some or all included benefits
• Elects not to continue receiving care at 208 Dental
• Does not schedule or complete included services before the membership year expires

Unused benefits have no cash value, do not roll over, and do not create a refund or prorated refund.

The Membership Savings Plan cannot be combined with dental insurance, another dental discount plan, or other coverage where applicable.

If you obtain dental insurance during an active membership period, you are responsible for notifying 208 Dental. Obtaining insurance does not create a right to a refund or prorated refund of an already-paid membership fee.

Each membership covers a 12-month period beginning on your enrollment or effective date. Included benefits reset on your membership anniversary date, not January 1.

Membership automatically renews annually using the authorized payment method unless properly canceled. A valid credit or debit card is required for enrollment and annual renewal.

We provide a renewal reminder approximately 30–60 days before renewal, including the renewal date, current rate, and cancellation information.

Written cancellation must be received at least 14 business days before the next annual renewal charge to stop that future renewal.

Canceling renewal does not cancel or refund the current already-paid membership year.

The member is responsible for scheduling and completing included services during the 12-month membership period.

Unused benefits have no cash value, do not roll over to a future membership year, and do not create a refund or prorated refund.

Yes. Membership pricing is applied first. CareCredit, Wisetack, or another approved financing option may then be used toward the resulting balance.

Use of approved financing does not reduce the applicable membership discount.

Treatment completed before enrollment is not automatically retroactively discounted.

If you enroll while an overall treatment case is still active or incomplete, membership pricing for that case may be determined by 208 Dental based on the timing and status of treatment.

Patients should ask our office how membership pricing will apply before enrolling during an active treatment case.

For orthodontic or other extensive/multi-stage treatment, membership must remain active for future treatment to continue receiving applicable membership pricing.

Membership pricing does not stack with another 208 Dental promotion, special, or discounted price unless specifically approved.

When more than one pricing option could apply, you will receive the applicable option that provides the better price rather than combining discounts.

Yes. Your diagnosis, hygiene frequency, and appropriate membership recommendation may change over time.

Included services must be clinically appropriate. Membership does not entitle a patient to a procedure that is not clinically indicated.

Services beyond the included quantities are billed separately at the applicable membership rate when eligible.

The 20% membership discount applies to eligible clinical procedures and treatment performed or provided and billed by 208 Dental. Retail and home-care products sold separately are not discounted unless specifically stated otherwise.

• Retail and home-care products
• Outside specialist fees
• Separate outside anesthesia or sedation-provider fees
• Administrative and non-clinical fees
• Late-cancellation and no-show fees
• Returned-payment fees and other administrative charges

Services referred to outside providers are not included in or discounted by the plan.

Benefits apply only to the services and quantities specifically stated for the selected plan. The appropriate treatment and hygiene schedule are always based on your clinical needs and diagnosis.

Treatment must be within the scope of services provided by 208 Dental. Standard 208 Dental scheduling, deposit, cancellation/no-show, and financial policies still apply to members.

Join the Membership Savings Plan within 30 days and the amount paid for your qualifying New Patient Special can be applied toward your annual membership.
